What actually changes

The chosen audio track is decoded and re-encoded as MP3. Video and subtitles are discarded.
Worth knowing first
MKV files often carry several audio tracks: original language, dub, and director's commentary. The first track is not always the one you want, and MP3 can hold only one, so the track has to be selected explicitly rather than by default.

What if the source is 5.1 surround?
It is downmixed to stereo. Dialogue lives in the centre channel, so a bad downmix makes speech quiet relative to effects.
Can I extract without re-encoding?
Only if the track is already MP3 or AAC and you keep that format. Converting to MP3 from AC-3 or DTS always re-encodes.
