What actually changes
PDF and DWG are not the same picture in two wrappers.
The PDF's vector content is read and rewritten as CAD entities. Lines stay lines and arcs stay arcs, so the geometry is snappable and measurable rather than a picture.
Worth knowing first
You get geometry, not a drawing. Layers, blocks, dimensions as live objects and the coordinate system are not in the PDF and cannot be recovered, so the result is a flat collection of lines that still needs rebuilding into a usable model.

What if the PDF is a scan?
There is no vector content at all. It needs raster to vector tracing, which is a different and much less reliable process.
Will it be to the right scale?
Rarely. Scale has to be set by measuring a known dimension in the drawing and scaling the whole import to match.
