What actually changes
CBZ and PDF are not the same picture in two wrappers.
Each image in the archive becomes a PDF page, sized to that image's aspect ratio, in filename order.
Worth knowing first
Comic pages vary in size, and double-page spreads are twice as wide as the rest. The PDF ends up with mixed page dimensions, which readers handle badly, so either accept the jumps or normalise the page size and letterbox the spreads.

Will the file get bigger?
Slightly. The JPEGs are carried through as they are, and PDF adds per-page structure on top.
Is the reading direction preserved?
No. Right-to-left reading for manga is a reader setting, not something stored in either format.
