A file downloaded years ago is an .rmvb and RealPlayer is long gone from every machine that matters.
RealVideo and RealAudio streams are decoded and re-encoded to H.264 and AAC in an MP4 container.
RMVB used aggressive variable bitrate at very low average bitrates, which is how a whole film fitted in 200 MB. That efficiency came out of the picture, so a conversion faithfully preserves heavy blocking and smearing. Do not raise the bitrate expecting improvement.
